Create and Manage Centrally Managed Forms

Note: This function is currently only available in a limited number of customer environments.

You can create a centrally managed form by uploading a PDF form in a central clinic location. These forms function similarly to other forms but are managed centrally. When a centrally managed form is updated in the central clinic location, the changes can be pushed to all local clinic locations where it is shared.

Centrally managed forms are automatically shared with local clinic locations as view-only. Users in local clinic locations cannot edit or delete centrally managed forms.

You can also create multi-context centrally managed forms, making the same form available in two or more contexts (for example, Patient and Consultation) across all shared clinic locations.

Requirements

  • A central clinic location is used in the system.

  • Access to the central clinic location is required.

  • 'Templates' user permission is needed to create centrally managed forms.

Create a centrally managed form

  1. Navigate to the central clinic location.

  2. Go to Settings > Lists & Templates > Templates > Certificates & Forms.

  3. Select the PDF form drop-down menu and select the form type. To make the form available in one context only, select a single context (Patient, Client, Consultation, and so on). The form type determines which placeholders are active and where the form can be used, for example on the patient page, the client page, or the consultation page. To make the form available in multiple contexts at the same time, select Multiple targets.

  4. In the Clinic locations field, select the clinic locations where the form will be available, or leave the field empty to make it available in all clinic locations.

  5. Follow the remaining setup instructions in Set up PDF forms.

  6. Select Save. The form is now available in the selected clinic locations. In the Certificates & Forms list, the form appears as view-only in local clinic locations.

Once saved, the form appears in the Certificates & Forms list in the central clinic location with edit and delete icons.

Tip: If you would like the clinic location logo on the forms, add an image field to your PDF and map it to the Clinic location's logo: [[department_logo]] placeholder in Provet. The logo is automatically displayed based on the clinic location where the form is used, which means this works also for centrally managed forms across multiple clinic locations.

Edit a centrally managed form

You can edit centrally managed forms only from the central clinic location. When you save changes, the changes are automatically reflected in all local clinic locations where the form is shared. For multi-context forms, you can choose which linked forms to update.

  1. Navigate to the central clinic location.

  2. Go to Settings > Lists & Templates > Templates > Certificates & Forms.

  3. Select the edit button next to the form you want to update. If the form is a multi-context form, select the master template to update all linked forms.

  4. Make the required changes and select Save.

  5. If the form is a multi-context form, the Propagation options dialog opens. Select which linked forms to update with the changes, then select Propagate to all forms. The changes are applied to the selected forms in all local clinic locations where they are shared.

Delete a centrally managed form

You can delete centrally managed forms only from the central clinic location. To delete a centrally managed form, navigate to the central clinic location, go to Settings > Lists & Templates > Templates > Certificates & Forms, and select the delete button next to the form. The form is removed from all local clinic locations where it was shared.
